In consideration of the above named player's entry into CGSL, I intend to be legally bound, or do hereby for myself, executors and administrators, waive, release, and forever discharge any and all rights and claims for damages, including any claims for loss of, damages or injury to her person or property arising out of her performance or failure of performance from the CGSL and it's members, managers, coaches, officials, sponsors, their agent(s), successors and assigns.
The above named player and I will comply with, and abide by the rules and policies of the CGSL , including, but not limited to the following; participants must play for the team assigned by the league. Players must participate in fundraisers. Non participants and participants who do not meet the minimum sale requirements in fundraising will be expected to pay an additional fee in addition to the registration fee. I understand that I am responsible for all monies owed the league. I understand that the CGSL does not carry health insurance on any player and is not responsible for any health costs.
